Physical Evaluation

Physical evaluation is done to determine the physical condition of an individual. Physical evaluations are mainly done by the governmental bodies or private organizations while recruitment to ensure that the person is physically fit for the job he/she have applied for. However, these types of evaluations are also conducted by doctors to ascertain the physical powers of donors. Although a primary and basic test, it helps in bringing out the various traits of the candidate/donor.

The candidate should answer the following questions with YES or NO only-

  1. Do you think you lead a healthy lifestyle?
  2. Are you aware of your BMI?
  3. Have you had a medical check since 12th January 2012?
  4. Do you think you have a fit body and physical condition?

The candidate should answer the following questions in detail-

  1.       Have you been a victim to any major diseases in the near past? If yes, elaborate.
  2.       Do you know your exact height and weight OR your Body mass index?
  3.       Do have any sort of physical illness for which you are receiving treatment?
  4.       Are you handicapped in any of your major limbs? I.e. Hands, arms, legs.
  5.      Do you have any sorts of vision problem? Do you wear spectacles? If yes, please write down the specifications.
  6.      Do you have any sorts of phobia that is related to your physical condition?
  7.      Are you allergic to anything? If yes, please elaborate
